As an Epic Hope Volunteer, I give permission to Epic Hope, Inc. and its affiliated organizations to use/reproduce/share images, videos, voice recordings, and/or stories of my experiences volunteering in Epic Hope promotional materials. I understand that these materials will not be used for the sale of any goods or services and that they are intended to inform and inspire individuals about the work of Epic Hope.

Background Check

As an Epic Hope Volunteer, I acknowledge and consent to undergoing a full background check as part of the volunteer screening process. I understand that this background check is necessary to ensure the safety and well-being of the children we serve. By volunteering with Epic Hope, I agree to provide any information necessary for this background check and understand that the results will be used solely for the purpose of safeguarding the children in our care.

Confidentiality Acknowledgement

As an Epic Hope staff member or volunteer, I understand and agree to uphold the highest standards of confidentiality regarding all interactions with program participants. I acknowledge that any information shared or discussed between myself and a program participant is to be treated with utmost discretion and respect for privacy. Confidentiality will only be breached in circumstances where mandatory reporting obligations arise, including but not limited to: • Suspected cases of child abuse or neglect. • Threats of harm to oneself or others. • Disclosure of illegal activities. In such instances, I understand that it is my legal and ethical responsibility to report the relevant information to the appropriate authorities, in accordance with applicable laws and organizational policies. By participating as a staff member or volunteer with Epic Hope, I commit to maintaining confidentiality as an essential aspect of fostering trust and safety within our community.
